The 2020–2023 Rural Health Network Development Grant Project: Putnam County Care Connect (PC3)

In 2020, the Rural Health Network Development Grant was awarded to the St. Johns River Rural Health Network and their partners to launch the Putnam County Care Connect (PC3) project. The purpose of this three-year project is to achieve efficiencies, coordinate and expand access to behavioral health services, and to strengthen the rural health care system as a whole in Putnam County, Florida.
